Your Appointment

Your first visit to Seacoast Dermatology, PLLC involves a few special steps so that we can get to know you. To understand what to expect, please read through this page.

HMOs and some other types of plans require referrals. Please be sure our office has received a referral (if required) prior to your visit.  The referral must come from the primary care provider on file with your insurance company for it to be valid and eligible for payment.  Otherwise, we will need to reschedule your appointment.

Please arrive 15 minutes prior to your appointment to check-in. Please bring a valid picture ID and insurance card.

Please be prepared to pay your co-pay and any coinsurance or deductible amounts OR for the full cost of the visit for non-covered services. If you are unsure of your copay or benefits, please contact the number on the back of your insurance card before your visit.

We recommend you save some time by printing and completing the New Patient Form below in advance of your appointment.

Insurance and Billing

We accept many insurance plans.  Qualifications for insurance coverage may differ due to the uniqueness of each procedure. As a courtesy, we will submit a claim for reimbursement if we are in-network for your plan(s).

We accept cash, checks, credit cards, and CareCredit. We also offer flexible payment plan options. Please see our Billing Coordinator for details.
